About Person Directed Supports
Welcome to Person Directed Supports where our goal is to provide essential resources and supports in a Person Centered, caring environment to individuals with intellectual disabilities and autism. Founded in February of 2000, PDS is proudly a family operated not for profit organization.
Our purpose is to support each individual using “Person-Centered Planning.”
What is Person-Centered Planning?
Person-Centered Planning (PCP) is a set of approaches designed to assist someone to plan their life and supports they want for their life. It is PDSI’s planning model to assist individuals to increase their personal self-determination and improve their own independence.
Person Centered Planning is used in Community Homes, Life Sharing, Behavior, Community Supports Programs, housing, in-home supports, supported living, companion living, and more to increase proactivity and a fulfilling life.
Our Mission
The mission of Person Directed Supports is to provide an opportunity for people and their families to learn about a larger world and to make decisions about how they choose to experience and contribute to the resources, joys and traditions of the community. Through a person centered planning process, comprehensive supports coordination, and best practice treatment and habilitative approaches; people will learn, grow and enjoy a wide array of new experiences, relationships and activity.
Our President
Pam Gibat is a longtime leader in human services with deep experience supporting individuals and families through person-centered care. Her work reflects a strong commitment to dignity, choice, and community inclusion, helping guide services that empower people to live fuller, more self-directed lives.
This focus aligns closely with Person Directed Supports’ mission of helping individuals with intellectual disabilities make choices, engage in their communities, and live with greater independence.
