Supporting Independence at Home and in the Community
Individuals in Supported Living are provided with direct and/or indirect services, living in their own homes or apartments, that are owned, leased, or rented by the person. Based on individuals’ Person-Centered Plans, they choose to move into their own home or apartment, and receive supports and services tailored to help them become as independent as possible. The specific support is discussed with the person and the person’s team, and provided for that person depending on what he or she chooses within his or her home.
Through this service, individuals can live in their own home and community and receive support to acquire, maintain, and improve upon skills necessary to live more independently and to be more productive and participate in community life. Services typically include general areas of self-care, health maintenance, wellness, meal preparation, home management, transportation, relationship building and socialization, and participation in community activities. The service provides 24/7 on-call assistance whenever needed.
